Through our partnership with Advanced Health SC, we offer Chronic Care Management (CCM) services designed to provide structured, ongoing support between appointments.

These services are covered by most insurance plans, including Medicare.

What Is Chronic Care Management?

Chronic Care Management is a Medicare-supported program for patients with two or more chronic conditions expected to last at least 12 months.

CCM provides:

  • Monthly care coordination
  • Medication oversight and reconciliation
  • Direct access to a clinical team between visits
  • Assistance navigating specialists and referrals
  • Ongoing monitoring of health status

This proactive approach helps reduce hospitalizations, prevent complications, and improve overall health stability.
